You should claim as soon as possible as you cannot usually ask for the claim to be backdated.
There are separate claim packs for children under 16, and for adults aged 16 or over so make sure you ask for the right one.
You can download a claim form for Disability Living Allowance (external website) for either someone aged under 16, or someone aged 16 or over from the Directgov website.
You can also get a form by telephoning the National Disability Benefits Helpline free on:

Or, by sending off the tear-off slip on the back of the Disability Living Allowance leaflet DS704 available from your local post office.
If you are blind or partially sighted and you want to get a claim pack in large print or Braille, you can order it from the Disability Benefits Helpline:

When you order your form from the Disability Benefits Helpline, or if you download a form from the Internet, it will be date stamped with the date when you made the request for the form. If you then return the completed form within six weeks and your claim is successful, your payments will start from the date stamped on the form. If you collect an application form from an Advice Agency such as your local Citizens Advice Bureau, your award will not start until after the completed form has been received by the Disability Benefits Centre.
